Membership Requirements
|
Membership is
open to any provider who is registered or listed with TDFPS. |
|
Annual
membership dues are $25.00 per year and are due each August.
For person joining after March 1st, dues will be $15.00 for
the remainder of the year. All dues are non-refundable.

Current copies
of CPR and First Aid Certificates, Registration Certificates and renewal checks
will be submitted to the Treasurer. Membership
applications will be submitted to the Referral Coordinator. |

All members
must abide by Texas minimum standards
and be in good standing with licensing. |
